There are four different ways to file a claim for reimbursement. We try to make it as easy as possible for you to submit claims and receive your funds.
Every FSA plan has a “Claims Filing Deadline” which is the last day to submit claims to get reimbursed from your FSA account. This deadline can differ from plan to plan depending on your employer. If you don’t know when your Claims Filing Deadline is, you can check your plan documents or give us a call to find out. Don’t leave money on the table! Ensure you’re getting the most out of your plan by filing claims.
1. Online – It’s easier than ever to submit claims online using the Proficient Connect online portal or the Proficient Connect App (download from the App Store or Google Play). It’s easy, convenient, and can even be done on the go!
~Simply log in, go to Claims & Receipts, and select Submit a Claim. ~Enter your claim details, upload your itemized receipt or Explanation of Benefits and submit for processing.
2. Mail – You can always mail us a completed and signed claim form along with your itemized receipt or Explanation of Benefits:

3. Fax – Feel free to fax us your completed and signed claim form along with your itemized receipt or Explanation of Benefits:

Avoid delays. Make sure your itemized receipt includes the following:
- Patient's Name: The name of the person who received the service or for whom the item was purchased. For retail store purchases, this information may be excluded.
- Provider's Name: The provider that delivered the service or where the item was purchased.
- Date of Service: The date when the service was provided or the item was purchased.
- Type of Service: A description of the service provided or the item purchased.
- Cost: The amount owed, paid, or portion not reimbursed through your insurance carrier.
Important: credit card receipts do not typically include the above requirements and will not meet the criteria to validate the eligibility of the expense. If you have any additional questions, our team is here to help!
